Thanks but building and fire codes don't really cover it and doesn't
feel like a confined space really, as it's just a room only accessible
by a ladder up through a hatch.

We solved it a different way - nearby roof access made it simple to
add a door at roof level - yes - out and in - but a warm climate where
there is rarely ever snow and ice.

I'm guessing best we can do is the 30 X 30+7 ladder clearance
requirements, though I see commercial roof hatches as small as 24 x 24
(but most common are 30 X 36).
