Whilst there are commercial - and pricey options out there, I simply built what I needed:

The velcro helps when pointing upwards. The sloped front allows for both horizontal or vertical mounting. The ¼ - 20 T-Nut for the tripod is positioned under the lens to allow even panning for 360 degree panoramas.
