NEC 210.11.C.3. Dwelling Unit Bathroom Branch Circuits "In addition to
the number of branch circuits required by other parts of this section,
at least 1, 120V, 20A branch circuit shall be provided to supply a
bathroom receptacle outlet(s). Such circuits shall have no other
outlets.
Exception: Where the 20A circuit supplies a single bathroom, outlets
for other equipment within the same bathroom shall be permitted to be
supplied in accordance with 210.23.A.1 and A.2."

So essentially, if the bathroom circuit isn't powering multiple
bathrooms, it meets code to power the bathroom lights and exhaust fan
from the same circuit. Also note that this requirement only applies to
guest rooms that can be defined as "dwelling units", which means it
must have permanent cooking equipment.
