I think you'd have to start considerably earlier, and with biiiiiiig 
infrastructure. You know, things like running water, enclosed sewers and 
water & sewage treatment plants.

The latest report I've seen says that Brazil's first-ever National 
Sanitation Plan, approved just three years ago, aims for universal 
access to drinking water by 2023, and universal access to sanitation in 
urban areas by 2033. Given the widespread poverty of much of the 
country, that's probably optimistic.
