A local production took it one step simpler: The bag full of "vomit" (in 
this case, plain water) was secreted inside a throw pillow, the actress 
started gagging and clutched it to her stomach, bending forward with 
appropriately disgusting vocal effects while squeezing it *hard*, 
resulting in a spew of water splashing on the floor at her feet. She had 
long hair that draped around her face so you couldn't tell the water was 
coming from the pillow and not her mouth.

This was in a tiny black box space, three-quarter round seating within 
feet of the actors, and from the audience's vocal reactions ( "Ewwww!" 
"Gross!") it was as realistic as it needed to be. And a super-simple 
cleanup.
