I faintly recall a local production did it with a reservoir (enema
bag-like) inside a pillow.... at some point, she got the tubing up in
her sleeve/hands across her mouth, while clutching the pillow and
squeezing.  See the previous post.... perhaps a combination can be
worked out sans pumps....
