In regards to revision tables. I think their main benefit comes when you include some symbols, like a triangle to indicate where the changes were made.  Another approach is to write a detailed description.  In the days of manual drafting, having multiple revision dates was perhaps useful. But, today, there is no real  benefit, as long as you keep the older versions.  The reason for the old versions is that sometimes you forget why you did certain design decisions, or made some changes later on.  The history sometimes reminds you of why the changes were made.

But, even in this case, there is no benefit in doing something only because it has been done that way elsewhere.  The policy needs to reflect an actual need, and be worth the hassle and time.
