What is likely happening is that the monitor is set to wake on signal and
actually bu the time it boots, is seen second while the Extron is always
powered and is seen by the Mac first.

if you can manually turn the monitor on first, this might help

This is why video servers such as the Hippotizer have a edid manager like
the DVI Parrot, Extron makes something similar, the ConnectPRO is also a
good option
