[SML] Blank forms for Scenery shifts

Hello All,

I need to teach High School students how to do several scenery shifts, and
I need to put one of them in charge of the process.
I would like the student leader to have a form for each shift, with
information about who moves what, on, or off, stage location, etc.

I have no problem in making up my own form, but I thought that one of you
might already have something like this in use that works for you.
Would you be willing to share what categories you are using, and/or what
order works well for recording and reading back scenery stage shifts.
