[SML] rope lock safety lock out tag

Bill,  that looks like a standard construction lock out tag. I've got two or three floating around in my tool box left over from doing install of ETC equipment over the years.  W.W. Granger lists several dozen types and styles ranging from about $4 to $35 and up.  They also have specialty types for gas cylinders, ladders, pneumatic systems and valve lockouts, etc.
