[SML] Blank forms for Scenery shifts

We have been using Google Sheets to keep everyone together. We had laptops
located on SL, SR, and with myself and the stage manager. Lighting and
sound really did not need them during the show but had access to the files
which helped keep us all together during tech. Follow spots had written
notes. Printed sheets were posted back stage as well in case of an internet
outage. The advantage with using the Google sheet method is that we were
able to update the document immediately for everyone. ASM's could assign
specific deck crew to do jobs and replace them on the document as
necessary. Working with high school students presents challanges of
consistency with crew members. Being able to replace people or add more was
easily done and it did not require me or a student Stage Manager to pour
over notes at the end of the night.

Columns on the sheet include:

Act and scene
Unit
Location
Spike Color
Cue
Crew Assigned

In addition to the columns we assigned different colors to each cell based
on what was happening to the set piece in question.

Green = set piece to spike
Red = strike set piece
Yellow = flown unit in
light blue = flown unit out
magenta = lighting effect
orange = sound effect
primary blue = preset piece

This is all part of a larger file which includes everything from script
analysis to production staff members, a line set schedule, budget info and
all sorts of other information for the show.
